Diagnostic X-Ray and Laboratory Services—90%

Benefits are provided for radiology (x-ray), pathology, laboratory tests, and other imaging and diagnostic services prescribed by a physician for the diagnosis of an illness or injury.

All services must be related to a definitive set of symptoms, except as provided for under "Physical Examinations" on pages 70-71.

Diagnostic x-ray and laboratory services performed or ordered by a chiropractor are not covered under this benefit (please see "Chiropractic Care" on pages 56-57).
