ID Card

Please present your Medical & Prescription ID Card to the provider's receptionist each time you visit a preferred provider. The ID card verifies your participation in the network, ensures direct billing, guarantees the preestablished discounted rates, and lists the telephone numbers required for inpatient preadmission certification and second surgical opinions. This card is not a guarantee of eligibility for benefits.

If you do not have access to the internet and need help finding a preferred provider or want to confirm that your provider is still in the network, please contact a Customer Service Representative at the Trust Office.
