Please contact the Trust Office any time you move. Your current address is required for us to process medical and dental claims, print Quarterly Benefit Statements and provide other materials and information necessary to administer your fringe benefits. When you contact the Trust Office, we will provide you with a Change of Address Notice By Participant. This form must be completed and signed by the participant and returned to the Trust Office.■
