Application

The 401(k) is funded with elective contributions through payroll deduction. A completed application is required to begin contributions or to change your contribution amount. Select one of the following applications and remember to forward a copy to your employer's payroll department.

Secure Application Use this online form if you are a registered user of our secure website. First, log in with your username and password. Then go to My Dashboard or Retirement Plans and find the link to Change Hourly 401(k) Amount. When you're finished completing the form, click the Submit button to transmit your application to Carpenters Trusts. You'll receive confirmation by email, and your application will be saved in My Documents so it can be downloaded or printed as needed.
Online Application If you are not a registered user of our secure website, use this online form to begin 401(k) contributions or change your contribution amount. Just complete the form and click Submit to transmit your application to Carpenters Trusts. A copy of your completed application will be sent to the email address provided on the application.
Paper Application Click to download a PDF version of our paper application. When connected to a printer, you can print the document, fill it out, and mail it to Carpenters Trusts. The PDF application can also be filled out electronically—just click on a field, enter the information from your keyboard or keypad, save the document, and email it to Carpenters Trusts.
