Confirm That Your Biometric Form Was Faxed

Did you already fax your biometric screening form? Read on to learn how to obtain confirmation that it was received.

Carpenters Health Plan members are required to submit proof of a health exam and biometric screening to qualify for this year's Carpenters Rewards incentive (a $200 gift card). Remember, the Biometric Screening Form can be completed by the health plan member or the member's physician. Either way, the form must be signed by the health plan member and faxed to Vivacity by Sept. 30, 2012. Instructions are provided on the form.

To confirm whether your form was received, send an email to ctww@vivacity.net and include your full name and date of birth in the body of the email. Vivacity will respond to your confirmation request within 2 business days. If you selected Option B on the Biometric Screening Form, you might also consider following up with your physician's office to make sure it was faxed.

A biometric screening is a simple blood test that measures cholesterol, triglycerides, and blood sugar.
