Biometric Screening Forms Due by Sept. 30, 2012
Final 4 weeks! Biometric forms due Sept. 30, 2012
A biometric screening is not something you should put off doing. It can reveal early warning signs of serious illnesses and health conditions before symptoms develop.
There's a reward for taking this important step to maintain good health. As well as helping improve your quality of life, a health exam and biometric screening are worth 75 points if you participate in the Carpenters Rewards incentive program.
A health exam and biometric screening is just one of four required activities for the program. Points are awarded for each completed activity. If you achieve 200 points by Dec. 31, 2012, you'll be rewarded with a $200 gift card that can be used in 350 ways.
If you're serious about qualifying for the incentive, make sure you turn in a completed biometric screening form by Sept. 30, 2012. The form was mailed to participants in June, but you can download it from CarpentersHealth.org (the EmbodyHealth website). Click on Carpenters Wellness Program to find the link.
You'll also find answers to frequently asked questions about biometric screenings, such as:
- "What are biometrics?"
- "Why does Carpenters want to know my biometric information?"
- "If I had a recent exam with blood work, do I need to do it again?"
- "Does my doctor have to sign and fax my form?"
- "When do I get my points?
Every activity matters when participate in Carpenters Rewards. If you skip this important activity, you'll fall short of the 200 points needed for a $200 gift card. You might be putting your health and well-being at risk, too.
Carpenters Rewards is available to active and retired (non-Medicare) carpenters, spouses, and dependent children (ages 19-25) covered by the Carpenters Health and Security Plan of Western Washington. To enroll, log on to CarpentersHealth.org (the EmbodyHealth website) and click My Rewards.
