Final 3 Days to Qualify for $200 Incentive

Sept. 30, 2012 is the last day to earn Carpenters Rewards points for getting a health exam and blood work (biometric screening). Like preventive maintenance on your truck, these simple procedures help you maintain good health for the long haul. On top of that, you can earn 75 points if your paper work is completed, signed and faxed by the deadline. Click here for the form and instructions.
Carpenters Rewards is a health and wellness incentive program for active and retired (non-Medicare) carpenters, spouses, and dependent children ages 19-25. The program awards points for worthwhile activities that help improve and maintain your health. Just four activities can earn enough points for a $200 gift card that can be used in 350 ways. Every activity counts, so the health exam/blood work requirement is a deal-breaker. If you miss this deadline, you'll fall short of the 200 points you need for a $200 gift card, even if you satisfy the other requirements.
Depending on the date of your last exam and blood work, you might be able to meet the exam/biometric requirement without scheduling a physician appointment. If so, you'll need to fax your lab slip with the required form. Forms with missing information and/or signatures are ineligible for points, so follow instructions. There's no time to submit a second form.
To participate in Carpenters Rewards, you need to be a registered user of the CarpentersHealth.org (the EmbodyHealth website from Mayo Clinic). If you're new to the site, you'll need to enter a special Rewards Number to register. Contact Participant Services by phone or email if you forgot or misplaced your Rewards Number.
