Form 1095-b Mailed to Participants

Watch your mailbox for your Form 1095-B, which lists the number of months you and possible dependents had eligibility under the Carpenters Health and Security Plan in 2018. Do not discard this form—you need this information to file your U.S. Individual Income Tax Return. Your Form 1095-B should be delivered by Friday, Feb. 22. Please open immediately and review carefully. If any of the information is incorrect, contact Participant Services (800-552-0635) so a corrected form can be issued.
