Form 1095-b for Tax Year 2023 Available Upon Request
Form 1095-B is the form used to report information about minimum essential coverage to the Internal Revenue Service. The form is a required filing under the Affordable Care Act. It is used to determine whether a taxpayer is liable for the individual shared responsibility payment. The IRS no longer requires taxpayers to report this information on their federal income tax return. Northwest Carpenters Trusts provided your information to the IRS as required. It is not necessary to print and mail Form 1095-B with your tax return. However, if you would like a printed copy of your Form 1095-B, please call (800) 552-0635 or email ps@carpenterstrusts.org during business hours (Monday-Friday, 8 a.m. to 5 p.m. Pacific).
