A Condition Management program from Envolve™ PeopleCare (formerly known as Nurtur) combines education and telephone counseling to help you overcome the challenges of living with a chronic health condition. The program provides information, resources and personal support to help you take control of your condition. Best of all, you don't have to do it alone-you're matched up with a personal Health Coach for guidance and support. This program is free of charge to eligible participants and dependents. Participation is voluntary, and it usually amounts to one or two scheduled calls per month, about 15 minutes each. Your privacy is assured, and your involvement will never be shared with your union, your employer, or anyone outside the administrative staff of the Carpenters Health and Security Trust.

The Carpenters Health and Security Trust will award a $200 gift card to eligible participants and dependents who complete a health assessment call and six additional calls with a Health Coach. This offer is available to new and currently enrolled participants in the telephone coaching program. A health assessment will determine whether you qualify for the program, and your Health Coach will determine the recommended frequency of your calls. There is no time deadline for completion of your calls. This incentive is available to eligible, non-Medicare participants, spouses, domestic partners, and dependents, including parents of non-adult children in the asthma program. There is a limit of one gift card per member, regardless of the number of programs enrolled.

Your award offer will be sent by mail after completing a health assessment call and six additional coaching calls with a Health Coach. Gift cards are provided by Hallmark Business Connections, offering a choice of 350 stores, restaurants, and other places to redeem your gift card. For fastest delivery, complete your offer form online (directions provided). Otherwise, return your completed form by mail. For added assurance, you'll be able to check your award status online or call a toll-free number.
