Affordable Care Act -- The Affordable Care Act ("ACA") requires employers to provide a Notice to employees regarding their current health plan coverage and the availability of insurance coverage through the Health Insurance Marketplace (commonly referred to as the "Exchange").

Application for 401(k) Contributions English | Español The employee can begin 401(k) contributions on the first day of employment. There is no waiting period to begin participation. The employee elects an hourly amount to be deducted from wages and returns completed form to employer's payroll office. Do not send the form to Northwest Carpenters Trusts. The employee may change the deduction amount once per month. A new form must be submitted to change the deduction amount.

Application to Rescind 401(k) Elective Contributions — To discontinue 401(k) hourly deductions, employee must complete this form and return it to the payroll office. Do not send the form to Northwest Carpenters Trusts.

Authorization To Transfer Contributions — When an employee is working outside the jurisdiction of their home trust and benefit contributions are being sent to a cooperating trust, this form is required to redirect contributions back to their home trust. The completed form should be delivered to the cooperating/away trust.

Authorization To Terminate Transfer of Contributions — When an employee is working in the jurisdiction of a cooperating trust and having benefit contributions sent to their home trust, this form is required to discontinue the transfer of contributions. The completed form should be delivered to the cooperating/away trust.

Contribution Agreement for Associate Employees (Sample) - An associate employee means an employee not covered by a collective bargaining agreement. A written contribution agreement between the employer and Board of Trustees is required for participation in a benefit plan.

Enrollment Application - Northwest Carpenters Health and Security Plan English | Español — To properly enroll in the Northwest Carpenters Health and Security Plan, the employee must complete this form in its entirety and return it to Northwest Carpenters Trusts. Our mailing address, fax number, and email address are on the revers side of the form. We cannot process the employee's medical claims or vacation contributions without a completed application on file.

Enrollment Application - Northwest Carpenters Retirement Plan English | Español — To enroll in the Northwest Carpenters Retirement Plan, the employee must complete this form and return it to Northwest Carpenters Trusts. Our mailing address, fax number, and email address are on the reverse side of the form. This form canalso be used to designate a new beneficiary.
