Life Insurance Payments 

Participant and Dependents

If you or a dependent are eligible under this plan and die from any cause whatsoever, your beneficiary receives the following life insurance payment:

Death of  
Participant $30,000
Spouse $5,000
Eligible children $5,000

You must designate a beneficiary for your life insurance on your Enrollment Application. If you fail to designate a beneficiary, your benefit will be paid pursuant to the terms of the insurance policy. You are always the beneficiary of your dependents’ life insurance benefits. A dependent is only covered under one participant.

Retiree and Dependents

If you or a dependent are eligible under this plan and die from any cause whatsoever, your beneficiary receives the following life insurance payment:

Death of  
Retiree $2,000
Spouse $1,000
Eligible children $1,000

You must designate a beneficiary for your life insurance on your Enrollment Application. If you fail to designate a beneficiary, your benefit will be paid pursuant to the terms of the insurance policy. You are always the beneficiary of your dependents’ life insurance benefits. A dependent is only covered under one retiree.
